Accident 19 November 2013 at Antwerp refinery in Belgium
On the 19th of November 2013 at 3pm, a steam explosion occurred at the Total Refinery, Antwerp Belgium.
The accident involved the boiler feed water system (operating pressure 70 barg (1,015 psi), operating temperature 280°C (536°F) of the continuous catalytic reforming unit (CCR, unit 72).
Two employees of a contractor company were killed instantaneously.
These two technicians were in the process of performing a leak sealing job on an existing leak box on the bonnet flange of a 16 inch motorized valve, when all twenty stud-bolts of the flange ruptured.
The bonnet and motor were launched into the process area, and landed at a 25 m (82 ft.) distance.
The investigation into the cause of the accident is ongoing.

Total veroordeeld tot boete voor dodelijk arbeidsongeval in Antwerpse raffinaderij
De correctionele rechtbank van Antwerpen heeft olieraffinaderij Total en Team Industrial Services schuldig bevonden aan het werkongeval waarbij twee arbeiders om het leven kwamen. De rechtbank legt hoge boetes op en de bedrijven moeten de nabestaanden ook een aanzienlijke schadevergoeding betalen.
Boezemvrienden German Duran Pacheco (30) en William Van Robbroeck (29) moesten op 19 november 2013 na hun geplande opdrachten nog een herstelling gaan doen op het Total-terrein aan de Scheldelaan in Antwerpen. Beide technici kwamen tijdens die opdracht om het leven bij een fatale ontploffing.
Het Openbaar Ministerie en de nabestaanden van de slachtoffers menen dat opdrachtgever Total en werkgever Team Industrial Services ernstige fouten hebben gemaakt en dat die hebben geleid tot het dodelijke ongeval. De rechtbank geeft hen nu grotendeels gelijk.
Drie beklaagden vrijgesproken
Total wordt veroordeeld tot een boete van 18.000 euro en werkgever Team Industrial Services krijgt een boete van 36.000 euro opgelegd. Beide bedrijven worden ook solidair veroordeeld tot het betalen van een schadevergoeding aan de nabestaanden van beide slachtoffers. Zo kent de rechtbank de nabestaanden een morele schadevergoeding van 24.000 euro toe.
Verder moeten de veroordeelde bedrijven de begrafeniskosten, het inkomstenverlies en de medische kosten van de nabestaanden vergoeden. De factuur loopt voor beide bedrijven op tot 90.000 euro.
De rechtbank stelt ook een deskundige aan die moet becijferen welke kosten de nabestaanden in de toekomst zullen moeten dragen. Drie beklaagden in de zaak werden vrijgesproken.

Total sentenced to fine for fatal work accident at Antwerp refinery
Antwerp's correctional court has found oil refiner Total and Team Industrial Services guilty of the work accident that killed two workers. The court imposed large fines and the companies must also pay substantial damages to the bereaved families.
Bosom buddies German Duran Pacheco (30) and William Van Robbroeck (29) were supposed to go to do another repair at the Total site on Scheldelaan in Antwerp on Nov. 19, 2013, after their scheduled assignments. Both technicians were killed in a fatal explosion during that assignment.
The prosecution and the victims' relatives believe that client Total and employer Team Industrial Services made serious mistakes that led to the fatal accident. The court now largely agrees with them.
Three defendants acquitted
Total is fined €18,000 and employer Team Industrial Services is fined €36,000. Both companies are also ordered in solidarity to pay damages to the relatives of both victims. Thus, the court awards the relatives moral damages of 24,000 euros.
Furthermore, the condemned companies must reimburse the funeral expenses, loss of income and medical expenses of the bereaved families. The bill runs to 90,000 euros for both companies.
The court will also appoint an expert to quantify what costs the next of kin will have to bear in the future. Three defendants in the case were acquitted.
